Requirements for Readmission and Transfer
07 July 2023 – Reminder to students intending to apply for transfer or readmission. Please take note of the following requirements before the deadline on 10 July 2023, Monday until 4:45 pm.
Submit the hard copy of your application documents in-person or send through courier to the Office of the College Secretary of the Degree Program you are applying to. You may also email the OCS for any queries or concerns at css-ocs.upbaguio@up.edu.ph
For Transfer:
T1: From Other UP Units
Note: Transferees from other UP Units must have completed at least 30 academic units.
Required Documents (Must submit two copies each of the following):
- Letter of Application (a letter addressed to the College Secretary of the student’s intent to apply for transfer)
- Application form with passport-size picture (Access the form at https://tinyurl.com/4vcshxn2 )
- Certificate of No Pending Disciplinary Case from the University
- Updated True Copy of Grades duly signed by the Registrar
- GWA Certification
- Notarized Certification of Non-enrollment (if there is a gap semester)
T2: From Other Schools and Universities
Note: Transferees from other UP Units must have completed at least 33 academic units with GWA of 2.0 or better (excluding P.E., Religion, and other vocational courses)
Required Documents (Must submit two copies each of the following):
- Letter of Application (a letter addressed to the College Secretary of the student’s intent to apply for transfer)
- Application form with passport-size picture (Access the form at https://tinyurl.com/4vcshxn2 )
- Certificate of No Pending Disciplinary Case from the University
- Updated True Copy of Grades duly signed by the Registrar or Authorized Signatory (Attach School’s Grading System)
- GWA Certification
- Notarized Certification of Non-enrollment (if there is a gap semester)
For Readmission:
Required Documents:
- Letter of Appeal with supporting documents (if necessary) stating reason/s for the appeal addressed to the Committee on Admission, Readmission and Student Progress (CARSP)
- Application form (Access form here: https://tinyurl.com/yz3j5p9m )
- True Copy of Grades (TCG)
Supporting Documents:
- Program of study
- Office of Counselling and Guidance (OCG) Certification [Note: Your appointment with the OCG will be scheduled after receipt of complete application documents.]
- Medical clearance stating that the student is fit to return to school, if dismissal is caused by medical issues.
- Endorsement from Thesis adviser if returning to finish thesis
- For Permanent Disqualification (PD), submit completed Referral Form (Access form here: https://tinyurl.com/abzwh6hz )
